It could be anything that requires you to send a fax back. Their request looks very legitimate. Scammers want the victims to send a fax back because the number given is a premium rate number, which charges sometimes $5/ minute. They are looking for the fax back to be long enough just so it takes a few good minutes for you to send it.

Research to see if the number provided is a hot-line. Also, check the initial document for fine print, which is required to be there on behalf of the premium line.
